• PARTICIPANTS MUST COMPLETE THE PADI MEDICAL QUESTIONAIRE AND IF REQUIRED HAVE THEIR PHYSICIAN’S SIGN-OFF INDICATING ITS SAFE TO PARTICIPATE

• Participants must not travel to more than 1000ft of elevation for 18 hours after this activity (including interisland and commercial flights, helicopter tours, skydiving, and trips to Waimea Canyon)
